Healthy Blue Patient Navigation and Person-Centered Planning Training

This is a two day workshop.  It will award 8.0 CE clock hours.  The first day is December  7, 2021  (11:45-5:00); the second day is December 15, 2021 (2:00-5:00)

Workshop provides 8 hours of training (in 2 parts) for participants who will be working with Medicaid patients with co-occurring SMI and physical health issues. Training is open to LPC’s, and PLPC’s as well as other participants, and will include person-centered planning.

Presenters will be 

Almarie Ford, LCSW, and Cheryll Bowers-Stephens, MD

Training will:

  1. Increase workers’ skills and abilities to reduce the number of patients who are high utilizers of hospital emergency rooms statewide.
  2. Increase workers’ communication skills with medical and behavioral health providers, and with patients during the engagement process.
  3. Increase workers’ abilities to assist their assigned patients in understanding their diagnoses and treatment options, as well as the resources available to them, including educating members about appropriate community services.
  4. Increase the workers’ understanding and knowledge about the differences between traditional and person-centered care management processes.
