This is a two day workshop. It will award 8.0 CE clock hours. The first day is December 7, 2021 (11:45-5:00); the second day is December 15, 2021 (2:00-5:00)
Workshop provides 8 hours of training (in 2 parts) for participants who will be working with Medicaid patients with co-occurring SMI and physical health issues. Training is open to LPC’s, and PLPC’s as well as other participants, and will include person-centered planning.

Training will:
- Increase workers’ skills and abilities to reduce the number of patients who are high utilizers of hospital emergency rooms statewide.
- Increase workers’ communication skills with medical and behavioral health providers, and with patients during the engagement process.
- Increase workers’ abilities to assist their assigned patients in understanding their diagnoses and treatment options, as well as the resources available to them, including educating members about appropriate community services.
- Increase the workers’ understanding and knowledge about the differences between traditional and person-centered care management processes.
