 Human Development: Early childhood is a time of rapid growth and development, and the foundations laid during this time have significant impact on health, education, and well-being across the lifespan. Expanded developmental surveillance and screening support early identification. The Helping Relationship: Care Coordination is patient and family centered team-based activity designed to assess the needs of patients, and link families with appropriate services and resources to achieve good health. Lifestyles and Career Development: Youth Health Transition prepares youth and families for a smooth transition into adulthood, especially youth with special health care needs

Please join Children?s Special Health Services on April 26th, 2018 for a day focused on the importance of developmental screening and surveillance, care coordination, and health care transition for children and youth with special health care needs. This event will be held at North Oaks Diagnostic Center in Hammond, LA. Attendance is free.
