Integrated Collaborative Care Model (ICCM) Integrated Collaborative Rounds

Are you a provider for Healthy Blue of Louisiana:

This workshop will present the integrated collaborative rounds processes with in the Integrated Collaborative Care Model, used by providers who are in value-based contracts with Healthy Blue of Louisiana.  In addition, the client identification and record review processes will be described in order to facilitate provider compliance with program requirements, and appropriate  documentation of follow-up activities.
