Overview - Integrated Collaborative Care Model (ICCM)

This workshop presents a brief overview of the Integrated Collaborative Care Model, which provides value-based incentives to providers who perform functions that extend the community tenure of clients who have challenging conditions, such as comorbid severe mental illness and major or chronic physical illness. Presenters are the Healthy Blue clinical team operating this evidence-based program.
